Patient Background and Initial Concerns

Jared, a 34-year-old from San Antonio, Texas, came to the San Antonio Gynecomastia Center looking for a long-term solution to a concern that had slowly taken over his confidence.

Over the past few years, he noticed increasing fullness in his chest. Despite staying active and maintaining a healthy weight, his chest looked soft, rounded, and out of proportion with the rest of his body.

This wasn’t just about aesthetics. Jared had become self-conscious in everyday situations, whether it was at the gym, at the beach, or even when choosing what to wear. He often layered shirts, avoided fitted clothing, and adjusted his posture to make his chest appear flatter.

Over time, these small adjustments became a constant mental burden. He wanted a chest that matched the rest of his build and no longer drew unwanted attention.

Looking at His Before Photos

Front View

Jared’s front-view photos clearly show a diffuse fullness across the chest. The tissue beneath the nipples formed a slight mound, which softened the natural definition of his upper torso. Instead of a flat or angular chest, his had a rounded shape that extended laterally toward the underarm area.

Side View

The profile view told an even clearer story. His chest projected forward noticeably, especially in the lower half. This degree of projection suggested a dense concentration of glandular tissue beneath the nipple-areolar complex.

Unlike fat, gland tissue has structural weight that physically pushes the chest outward. This observation helped guide the surgical plan, as liposuction alone would not be enough.

The Surgical Approach

Jared’s surgery was designed to address both the fatty and glandular components of his gynecomastia. A two-step approach ensured a complete correction and natural-looking result.

Liposuction to Remove Fat

The procedure began with power-assisted liposuction to reduce the fatty tissue across the chest and into the lateral regions. This helped remove volume and improved the overall contour, especially in the outer chest and underarm areas.

Glandular Excision for Definition

Through a small incision placed along the lower edge of each areola, I carefully removed the dense glandular tissue. This step is essential in cases like Jared’s.

Gland removal provides a flat, defined result that liposuction alone cannot achieve. The incision site was chosen to allow the scar to blend in naturally over time.

Skin Retraction Without Excision

Jared’s skin quality was excellent. Because of his good elasticity, no skin needed to be removed. With proper compression and time, the skin was expected to retract on its own and conform to the new chest shape.

His Healing Process

Jared’s recovery followed a smooth and steady timeline. In the first few days, he experienced typical swelling and bruising, which were managed with compression and rest. He wore a compression garment continuously, which helped reduce fluid buildup and guided the skin to settle evenly.

By the two-week mark, the swelling had decreased enough for the new contour of his chest to become visible. His side profile had already changed. The chest no longer projected forward, and the puffiness beneath the nipple had resolved.

By four weeks, Jared had returned to normal daily activities and light exercise. I advised him to hold off on chest-specific workouts like push-ups and bench press until at least six weeks post-op to protect the internal healing process.

His Final Result

The final results reflect exactly what we aim for in gynecomastia surgery, which is a natural, masculine definition without visible signs of surgery.

His front view now shows a flat, contoured chest that matches the rest of his body. There is no excess volume beneath the nipples, and the transition from the chest wall to the sides is smooth and athletic-looking.

The side view shows the biggest difference. The former projection is gone, and the nipple-areolar complex lies flush against the chest. His scars, placed along the lower border of the areolas, are subtle and will continue to fade with time.

But most importantly, he no longer feels the need to hide. He wears the clothes he wants, moves with more confidence, and no longer fixates on how his chest looks throughout the day.
