This patient came to the San Antonio Gynecomastia Center looking for something simple. He wanted to feel more comfortable in his body. His chest had always been a point of frustration, and he was ready to fix it.
From the first look at his photos, you can see why he felt that way. His chest had a full, rounded shape with noticeable projection beneath the nipples.
Even though he wasn’t significantly overweight, the contour of his chest gave the impression of breasts. It was something he had learned to hide with loose clothing and posture. But he was ready to stop hiding.

Evaluating His Chest
What stood out most in his exam wasn’t just the fullness. It was the firmness of the tissue beneath the nipple and the way the chest seemed to push outward even when relaxed.
That kind of contour typically means glandular tissue is playing a major role, not just fat. In his case, it was also clear that the skin had been stretched slightly over time, softening the shape of the chest and making it appear less defined.
He didn’t need a dramatic transformation. He just needed a careful, well-planned correction that matched his body and looked natural.
How the Surgery Was Done
During surgery, I began with liposuction to remove the excess fat along the upper and outer chest. This helped smooth the contour and reduce overall volume.
Once that was complete, I used a small incision around the lower border of the areola to remove the dense glandular tissue underneath. That tissue cannot be suctioned out. It has to be removed directly to create a flat, masculine chest.
Since his skin quality was still relatively strong, I did not need to make any large incisions or remove skin. I expected the skin to contract well once the underlying tissue was gone.
His Healing and Final Outcome
The first week after surgery brought the typical swelling and bruising, but he followed his instructions closely and wore his compression garment every day. As the swelling faded, the result became more and more visible.
By the six-week point, he was already wearing tighter shirts and standing straighter. At the three-month
follow-up, his chest looked balanced and proportional.
The nipple area now sat flush against his chest wall. There was no bulge, no projection, and no loose skin hanging over. The scar around the areola was healing well and was already hard to see in normal lighting.
What made this result so satisfying was its subtlety. It didn’t scream “surgery.” It looked like how his chest should have always looked.
